Learning Session Synopsis:

Participants will come prepared with some pre-work prepared in advance, and will be introduced to the change package.  Participants will learn the Model For Improvement as developed by the Institute for Health Care Improvement (IHI) and other strategies for implementing change in healthcare settings.  They will return home to begin the process of improving care in their ED, with continuing support from IBHI.  They will immediately begin to share data through IBHI with others in the collaborative and will engage in a continuing process of mutual support and improvement, via electronic media.  There will be a second learning session to further improve the change process approximately three and one half months later, followed by continued sharing of data and mutual support and then a final learning session in another six months to complete the collaborative.  Participants will continue to share data and provide mutual support for another six months following the close of the collaborative.

Participating Organizations Are Expected To:

·                   Obtain commitment from senior hospital and Emergency Department leadership to the completion of the collaborative

·                   Perform pre-work activities to prepare for the first Learning Session;

·                   Connect the goals of the Pioneering Breakthrough work to a strategic initiative in the organization;

·                   Provide a senior leader to serve as sponsor for the team working on the collaborative, serve as champion for the process, and attend at least the Second Learning Session;

·                  Send a team to all Learning Sessions;

·                  Provide resources to support their team including resources necessary for Learning Sessions, time to devote to testing and implementing changes in the practice (sufficient released FTE for the duration of the Collaborative) and active senior leadership involvement;

Learning Session Attendees: Who should come to Learning Sessions?

Choose the three individuals who can work together effectively, learn the methodology, and plan for action upon returning to your organization. At least one person should be a leader of general emergency department care and one person should be a leader of emergency department behavioral health care.  These three representatives need not be the same individuals for each Learning Session, although teams generally find it helpful when they are. We have developed the following guidelines from past Collaboratives to help you determine who should attend the Learning Sessions.

·        The day-to-day leader(s) should be at all three Learning Sessions.

·        The physician champion(s) should be at all three Learning Sessions.

·        The technical expert, if different from the physician champion, should be at all three Learning Sessions.

·        The system leader should be at the first Learning Session, but it may not be critical for this individual to attend other sessions.

·        A member of the senior leadership team, preferably the senior executive who is sponsoring the team should be at the first Learning Session.

Costs:

The cost of this collaborative is $13,500 and includes annual membership ($2,500) in IBHI.  Options are available for additional support through co-sponsorship and or Charter Membership.   The fee for the collaborative includes participation in each of the Learning-innovation sessions for three members of team or hospital leadership.  Additional members can be included at cost.  Participants will pay their own travel, lodging and incidental costs for learning sessions. To obtain more information, call (518) 732-7178.

Deadline for enrollment is December 14, 2007.
